Please complete and submit this form to request information about objects in the Architectural Fragments and Models, Costume and Textiles, Decorative and Industrial Arts, and/or Paintings and Sculpture holdings of the Chicago History Museum collection. This form may also be used to request an in-person research appointment to view materials in the above holding areas. Please note that, due to the extensive resources required to handle and retrieve this category of material, access will be limited to researchers who are able to demonstrate that in-person access is required to support substantive research and that the results of that research will be disseminated widely and thoroughly.
